Hey all,

I have been trying for 2 days to edit this form. It has everything I need except a few missing fields for extra data. If someone could help give me a nudge along I would greatly appreciate it! Basically what I’m looking for is to add a few fields below the "from email" field. If anyone could help out I would greatly appreciate it!
